- For sponsors, the wife or husband of an Indonesian citizen, attach a marriage book, sponsor's ID card, sponsor's family card and a marriage certificate from the relevant agency (minimum marriage period is more than 2 years);

- For foreign workers, attach the latest IMTA and RPTKA as well as other company documents (TKA as the highest leader in the company and ITAS holder for more than 4 (four) consecutive years)
- For Foreign Investors (PMA) attach a Recommendation from the Investment Coordinating Board (BKPM), company documents and more than 4 (four) ITAS holders consecutive years)
